Complex Care Coordinator Inspira Health Vineland, NJ $10,000 Sign on Bonus The region's leading network of healthcare providers, Inspira Health is seeking a full-time Complex Care Coordinator to join us at our Vineland, NJ location. You will play a critical role in collaborating within the interdisciplinary team to perform care coordination for high-risk patients and ensuring continuity of care and quality healthcare delivery across the care continuum. Responsibilities include: Providing psychosocial assessments on patients and continuously assessing the required health care, educational and psychological needs of the patient and family Utilizing risk stratification tools to assess chronically ill populations and deliver interventions to prevent progression or exacerbation of illness Overseeing complex cases in collaboration with the Inspira Health care coordination team, behavioral health team and other healthcare providers Communicating findings to the primary care provider, care coordination team and healthcare providers to avoid hospitalization and/or emergency room visits, with a focus on providing patient self-management support to empower the patient and family Improving quality of care, patient experience and throughput with the highest efficiency and shortest length of stay Obtaining authorizations for services from insurances to facilitate the patient's timely transition through the care continuum Initiating, participating and facilitating family meetings to help enhance communication regarding complex case needs, as well as referring patients and families to available community support services if necessary Qualifications: Master's in Social Work from CSWE accredited School of Social Work required State licensure by the NJ State board of social work examiners required and a Licensed social worker (LSW) required; licensed clinical social worker (LCSW) preferred 3+ years of experience in post-master's in social work in an acute care setting Certification in Aging & Health, Addictions, Behavioral Health and/or Palliative Care preferred Find your inspiration.
