Care Coordinator to join our team supporting.This is a remote
opportunity for experienced Registered Nurses who are passionate
about coordinating patient care, improving health outcomes, and
supporting members through effective care management. Requirements:
- Active Pennsylvania RN License OR Nurse Licensure Compact (NLC)
that includes Pennsylvania - Candidate must live in PA, NJ, or DE -
Strong care coordination and case management experience preferred -
Excellent communication and organizational skills Assignment
Details: - Duration: 3 months (possibility of extension) - Pay
Rate: $45/hour - Shift: Day shift - Work Setting: Remote If you are
