This position requires regular work at our Philadelphia office and
some travel within the local community. We do not work remotely.
ABOUT APM & CUA Asociación Puertorriqueños en Marcha (APM) has been
helping families achieve their greatest potential since 1970 by
providing early childhood education, foster care & adoption, child
welfare, mental & behavioral health, community and economic
development, housing, community school services, and violence
intervention services to the North Philadelphia communities. The
Community Umbrella Agencies (CUA) are community-based agencies
established to provide case management services and ensure the
permanency, wellbeing, and safety of the children, youth, and their
families. APM CUA is looking for compassionate and committed social
workers who want to create long-lasting impacts in the lives of
children and families. APM CUA 2 services the Lower Eastern North
region and CUA 5 services the Logan/Olney region. JOB SUMMARY The
Well-Being Specialist serves as specialist focused on physical,
mental and dental health as it relates to the children and youth,
we service. The Well Being Specialist works specifically to achieve
health and wellbeing related requirements and outcomes. The
Well-Being Specialist works as part of the CUA Case Management Team
to assist with achieving the medical objectives in the Single Case
Plan (SCP). The Well-Being Specialist works with families at
designated home visits to help parents establish positive
parent-child relationships, to support child safety across
environments, to promote healthy child growth and development and
to support the emotional well-being of children and families. The
Well-Being Specialist assists the family in identifying a primary
health insurance, accessing the insurance, identifying a primary
doctor and accessing that service, identifying a dentist and
accessing that service. Key job tasks/duties/responsibilities of
Well-Being Specialist Acts as a liaison between DHS Medical Unit
Ability to demonstrate working knowledge in the following topics:
infant care, child health and safety, maternal and family health,
infant and child development including language and literacy, role
of culture in parenting, child abuse and neglect, child trauma and
early brain development, and supporting the parent-child
relationship preferred. Assist with making sure that all children
and youth involved with the CUA receive timely medical, dental, and
behavioral health examinations and interventions. Medical and
dental appointments must be completed at the prescribed intervals.
Coordinate, track, and monitor these medical services and
appointments for families at the direction of the CUA CM or
Supervisor, or both. Transport children, youth, and families to and
from these appointments as directed by the CUA CM or Supervisor, or
both if necessary. Consult with DHS Nurses, when necessary. Consult
with the DHS Medical Eligibility Unit (MEU) and the DHS Health
Management Unit (HMU) as needed to ensure continuity of medical
insurance and care. Attend Court Hearings as needed but never in
lieu of the CUA CM. Document any activity performed per policy and
ensure all documentation is in the appropriate section of the case
file May be responsible for other duties as assigned by the
Intervention Supervisor as needed. BENEFITS Health Insurance

equivalent is acceptable; however, a bachelor's degree is
preferred. Proficiency in English and Spanish preferred. Strong
organizational skills. Demonstrated written and oral communication
skills Demonstrated ability to work independently and as a team.
Ability to set priorities in critical situations. Strong Customer
Service insight. An applied understanding of general ethics and
confidentiality. Maintain a high degree of discretion dealing with
confidential information. Basic proficiency in Microsoft Office
