In this role, you will provide support to Nurse Practitioners and
patients in long term care settings; support local markets in
launching new facilities; provide coverage to local market Nurse
Practitioners; deliver high quality care on-site. This is a
flexible, autonomous role that creates enormous satisfaction for
the NP as you impact the care and comfort of our aging population.
All the while, you'll be building meaningful relationships with the
residents and their families and the health care providers who are
responsible for their care. -

This position requires travel through the State of New Jersey.
Travel between care sites is based on the needs of business /
market and required for the role. Expected Travel 75% - 100%

Primary Responsibilities:
Obtain and review medical history, conduct assessments, analyze and
diagnose conditions, and develop appropriate plan of care
Identify gaps in care and interpret diagnostic test / reports in
order to refer appropriately
Identify risk factors and help mitigate barriers to accessing care
to reduce risk
Develop interventions to assist members in attaining established
goals of care
Evaluate member's progress in completion of goals of care and re -
assess and assist in care management with members
Serve as a key resource on complex and / or critical issues to
solve problems and develop innovative solutions
Establish trusting relationship and maintain communication with the
member, family / authorized representative and primary caregiver
and specialists
Assist members / families in understanding medical options /
interventions and in making informed decisions
Clarify member's cultural values that may impact health management
/ decisions
Identify prognosis / trajectory of chronic disease that may impact
future health decisions and conduct advanced care planning
discussions

Knowledge, Skills, and Abilities: -
Ability to spend at least 1 hour with a member in their home, which
may be in understaffed or remote areas, in the presence of pets or
individuals who are tobacco users
Ability to lift a 30-pound bag in and out of car and to navigate
stairs, as needed
Ability to travel 75-100% of the time for field-based work
Excellent administrative skills and the ability to utilize
technology, such as navigating the Internet on a computer
Excellent organizational skills with the ability to multi-task and
manage schedule
Excellent collaboration skills and ability to effectively
communicate with seniors, their families, their care team, and
physicians

Qualifications:
Active, unencumbered APC license to practice in the State of New
Jersey (or ability to obtain)
Active Nurse Practitioner certification through a national
board:
Board certified through the American Academy of Nurse Practitioners
or the American Nurses Credentialing Center, with certification in
one of the following:
Family Nurse Practitioner -
Adult Nurse Practitioner -
Gerontology Nurse Practitioner -
Adult-Gerontology Acute Care Nurse Practitioner -
Current, active DEA licensure/prescriptive authority or ability to
obtain post-hire
Ability to travel 75% - 100% time including overnight
travel

Preferred Qualifications:
1+ years of experience with hands on, post grad Nurse Practitioner
work
Experience in geriatric or adult medicine
Experience working with complex patient populations, including
multiple chronic diseases with diverse psychosocial backgrounds
Experience and comfortable working in LTC, home health or in home
setting independently
Understanding of Medicare, Medicaid and Health Plan benefit
structures beneficial
Bilingual or multilingual

New Jersey Residents Only: -The salary range for this role is
$88,000 to $173,200 annually. Pay is based on several factors
including but not limited to local labor markets, education, work
experience, certifications, etc. UnitedHealth Group complies with
all minimum wage laws as applicable. In addition to your salary,
