Job Description: Nurse Practitioner

One of our largest clients is seeking a nurse practitioner in Atlantic City, NJ. In this role you will be responsible for improving the overall quality of care, decreasing the need for avoidable hospitalizations, and supporting the members in developing a plan of care consistent with their overall goals of care. The NP partners with the member's primary care physician, the member/responsible party, and nursing home staff to provide primary/preventive care and personalized care coordination while proactively identifying triggers for acute illness and managing chronic conditions.

Hours:
---Monday through Friday 8AM - 5PM

Key Responsibilities:
---Assist case management in the evaluation of selected long-term patients.
---Follow up with ""new"" and previous long-term patients.
---Provide general medical care and treatment to patients in skilled nursing facilities within the guidelines and instructions.
---Ensure proper illness and injury care and disease prevention, diagnosis, treatment, and recovery.
---Perform and document a complete history and physical appropriate to the patient's level of illness and complexity.
---Formulate differential diagnoses and evaluate new and existing patients.
---Provide preventative and primary care for patients in assigned long term care facilities.
---Maintain accurate and thorough data of all patient records.
---Advise patients regarding their health and illness prevention.

Qualifications:
---Accepted license types, Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care.
---Active RN and NP license in Pennsylvania
---Reliable transportation
---One plus years of Nurse practitioner experience.
---Experience in long term care and geriatrics preferred.
---Strong communication and interpersonal skills
---Ability to work collaboratively with patients, families, and healthcare team members.
